Pathway Planning

Pathway Planning is a collaborative, person-centered alternative to traditional transition assessments. A team of professionals from varying disciplines works together with the student, family, and school team to create a personalized plan that connects strengths, interests, and real-world goals across five distinct domains of daily life. The result is a comprehensive, individualized plan with actionable steps and local resources.

The supports, accommodations, and environmental adaptations that are needed to help an individual feel comfortable and successful.
