Pathway Planning
Pathway Planning is designed for any individual with a disability who is preparing for a major life change. Whether planning for graduation, employment, independent living, or increased community involvement, this in-depth, person-centered assessment helps individuals and their families make informed decisions about the future. Our skilled team works closely with you and your family to understand your goals, aspirations, and preferences.
Our model brings together experts from various fields, including behavior analysis, occupational therapy, speech and language, assistive technology, college and career counseling, vocational coaching, and more. This multidisciplinary team collaborates to gather and analyze data.
Using this insight, we create a customized Pathway Plan that outlines clear, actionable next steps aligned with the individual’s vision for the future and connects families with local resources and supports. Our Pathway Planning service is currently available in all 50 U.S. States.

Comprehensive Transition Assessment
Our Comprehensive Transition Assessment helps transition-age students with disabilities and their families plan for the future with clarity and confidence. This detailed, person-centered assessment explores the student’s strengths, needs, interests, and goals related to daily living, community participation, postsecondary education, employment, and overall quality of life.
We work closely with students, families, and school teams to gather information through interviews, observations, and tailored assessment tools. The final report includes practical, individualized recommendations that help guide IEP planning, connect students to community resources, and set meaningful goals for adulthood.
This service can be funded privately by families or by school districts as part of transition planning. Comprehensive Transition Assessments are currently available in Connecticut and Hawaii.

Transition planning is a process centered around supporting young adults with disabilities as they reach pivotal points in their education and beyond. The process of planning for employment, postsecondary education, and daily living/community participation can begin as early as middle school, where goals, preferences, and interests are established. As individuals reach the maximum age of eligibility and/or graduation, the need for transition planning and assessment becomes even more important.

Pathway Planning, our version of transition planning is designed for any individual ages 12 - 30 with a disability recognized under IDEA or the ADA.

The process begins with an initial consultation where your transition consultant will collect information helpful in planning our assessment and data collection. Our team then begins to collect data through interviews, observations, standardized assessments, and informal inventories. After data is collected, it is reviewed by the multidisciplinary team and a comprehensive Pathway plan is developed.

Pathway Plans can be extremely beneficial for families and educators in informing the IEP process related to transition planning. Our staff have years of experience supporting students in the public education system and are well versed in special education law and policies/procedures.
